Off sick less than 7 days
If you are off sick for less than 7 days your employer cannot ask you for medical evidence of illness, but they may ask you to confirm you have been ill. This is called self certification.
Some employers will have their own self-certification forms for you to complete, while others may ask you to use the SC2 form from HM Revenue and Customs - Employee's Statement of Sickness.
Off sick more than 7 days
If you are off sick for over 7 days, your employer will typically ask for a fit note (Statement of Fitness for Work) from a GP or hospital doctor. Fit notes can someotimes be referred to as a "medical statement" or a "doctor's note"

When your fit note runs out
If you are still ill after the end date of your fit note, please contact us again via the online triage form to be considered for an extension.
